How to: fill in forms for Jamaica parish Court: Recovery plaint note




HERES a Plaint Note FORM with guidelines to follow in filling in your own plaint note form for any of the Jamaica’s Parish Court.
Please bear in mind that each parish court office has its own form and this variation is prepared with reference to the master form.
You therefore can either use this one or the one that you can collect from the court. The ones prepared by the Court are distributed for free.

A reminder: Forms are not the end all of a court matter.
After the forms are filled in and submitted, you will be given a receipt. You are expected t o return a few days later with the receipt to collect that summons to give to the other party.
Follow closely all instructions given about how to serve and return proof of service of the summons and any other document.
