So, you have served a notice to the tenant or the licensee or the squatter and now that that notice has expired, you are rearing to go to the Courthouse to get an order for recovery there.
The Evidence Form For Recovery of premises
Is the third form that you must fill out when filing a claim at the lower courts in Jamaica renamed as Parish Court in 2016. Important to note that you must fill out three copies of this form or if you make three copies, make sure you sign each individually. This site gives more information on how to fill this form out.
Below is an Evidence form which can be used, but you must first download it. Otherwise you may collect three with your free package of application forms from any parish court.
